​In my capacity as a parent / guardian, hereby I give permission to my child(ren) that I named below, referred to as “my child” in this form, becoming a member of/ and to train at Gymnastex Dubai (License no 664193).

• I am completely aware and understand that Gymnastex staff members are not physicians or medical practitioners of any kind what so ever. Keeping that in mind, I hereby release the Gymnastex staff to provide temporary first aid to my child/children in case of any injury or illness occurred to my child/children, and if found necessary by the Gymnastex staff to call and ask for medical assistance which includes transportation by a Gymnastex staff member or it’s representatives, whether paid or volunteer, to any healthcare or hospital, or calling of an ambulance for my child/children should the Gymnastex staff believe this to be necessary.

• We, the staff of Gymnastex, know our responsibilities to make our students and their parents/guardians attentive of the risk and hazards linked with the Gymnastics Sports, tumbling, trampolining and dance. Students may suffer injuries, possibly minor, serious or catastrophic in nature. 

• Parents/Guardians should make their children/wards aware of the possibility of injury and encourage their children/wards to follow the Gymnastex rules, regulations and the coaches’ instructions.

• Gymnastex, its coaches and other staff members, will not accept responsibility for injuries sustained by any student during the course of gymnastics, tumbling, trampolining or dance instruction, or open workouts, or in the course of any exhibitions, competitions or clinics in which he or she may participate or whilst traveling to or from an event.

• With the above in mind and being fully aware of the possibility of injury involved, I consent to my child/ward or children/wards to participate in the programs offered by Gymnastex. I also affirm that I now have and will continue to provide proper hospitalization, health and accident insurance coverage, which I consider adequate for my child’s/wards protection and my own protection.

• I also understand that it is the parent’s/guardian's responsibility to warn the child or children about the dangers of the above-mentioned activities and injuries that could occur. I will also ensure that my child and all persons accompanying my child will obey the Rules and regulations while inside the premises. I understand that Gymnastex reserves the right to disallow access to its premises if my failed to obey the Gymnastex Rules and Regulations and/or instructions given or if Gymanstex considers that my child is incompetent and/or an immediate risk to others.

• I waive, release, and discharge liability to Gymnastex, its employees, volunteers and representatives for personal injury or any accident caused as a result of my child’s negligence or as a result of my child disobeying instructions and/or the Rules and regulations whilst participating in an activity/event.

• I certify that my child is physically fit and I am aware of no physical reason, illness or medical condition to which would prevent my child/children from participating in Gymnastex classes or events.

• I understand that no responsibility will be accepted by Gymnastex for any loss or damage to the property or valuables of my child/children.

• I understand that photography and video-photography is not allowed in the premises during normal training sessions, except by Gymanstex members of the staff or a professional photographer hired by Gymnastex or representative.

• I understand that it is likely there will be photography and/or video-photography taking place and that Gymnastex has the irrevocable and unrestricted right to use and publish this material for any purpose (including social media, editorial and advertising).

By submitting below, I acknowledge that I have read the above and understand its contents; that I am at least eighteen (18) years old and fully competent to give my consent; That I am aware of the risks involved and give my voluntary consent in signing it as my own free act and deed; with full intention to be bound by the same, and free from any inducement or representation. By signing below, I agree to each statement above and release Gymnastex Sports Services from any and all liability for the unintentional exposure or harm.
